As for the second module Assignment:
(1) Have you ever identified an ADR?
Answer is YES.
(2)Have you ever reported an ADR to the appropriate authorities?
Answer is YES
(3) Which pharmacovigelance center are near your current workplace to fercilitate ADR reporting.?
Answer is the very Hospital where I am working presently ,as I was given the ADR form by the pharmacy Board of Sierra Leone to be doing drug mornitoring and be observing the safety of the drug use at our facility.and I will finally send The report to the regoratory body which is the pharmacy Board of Sierra Leone at the pharmacovigelance unit.
So after my Hospital for reporting
Second is the pharmacy Board of Sierra Leone.
As for the second Assignment:
(1)What would you recommend for better ADR reporting?
Answer:
(1) Training or educating of the health care professionals on how to recognize and report adverse drug reaction by providing information on common symptom and how to differentiate between an advers drug reaction and other condition and to know the important of reporting also open communication between health care professionals, patients and regoratory body.
(2)How accessible are the ADR reporting forms and pharmacovigelance center near you ?
Answer.,:the accessibility is through the regoratory body in which they have the department and they provide the link or site where we normally do reporting through internet or Emailing in form of a forms to fill to fill with the nessisary reporting system
